passionate and dedicated service. We have full time and part time opportunities
available working Monday to Friday across our communities. With us you will be

As a Health
Visitor, you will hold continuing responsibility for a defined caseload of
children aged 0-5 years and their families, delivering high quality care across
community, universal, targeted and specialist levels of need.
You will
play a pivotal role in supporting families through evidence based interventions
and early identification of need whilst leading the delivery of the
Healthy Child Programme (05 years). This role offers the opportunity to make a
meaningful difference to children’s lives by promoting health, reducing
inequalities, and supporting families at every stage of their journey.
You will
contribute to the realisation of the outcomes for North East Lincolnshire, in
particular to enable all people in NEL to feel safe and be safe, to have good
health and wellbeing and to realise their potential through skills and
learning.
This will be
achieved through the effective delivery of a range of support to young people
and families in the locality, in order to improve their health and wellbeing
and reduce the need for statutory intervention.
Children’s Public Health Nursing Services aims to ensure that all children, young people and families in North East Lincolnshire receive the full service offer (The Healthy Child Programme 0-5 years).
This includes universal access and early identification of additional and/or complex needs, with timely access to specialist services, to secure local services that enable Health Visiting and School Nursing teams in partnership to contribute to improve our outcomes and reduce health inequalities for children, young people and families.

Purpose of the Role:

1. Contribute to the delivery of a 0-19 years programme with specific focus on
the delivery of the Health Visitor-led Healthy Child Programme 0-5 years
2. Management and continuing responsibility for a designated caseload within an
agreed caseload model for children aged 0-5 years and their families requiring
community, universal, partnership and partnership plus interventions.
3. Prioritisation of conflicting demands
4. Work as part of a multi-disciplinary/agency team with allied health and
social care professionals, individual clients, groups and communities.
5. Manage caseloads including vulnerable children and attend Child Protection
case conferences.
6. Act as Nurse Prescriber (if qualified).
